MEMORY SERVICE

This provides the first step in the assessment process, including an appointment with a psychiatrist and possible referral to other agencies.

COMMUNITY MENTAL HEALTH NURSING TEAM

This team provides an ongoing assessment of, and support for, the person with mental health/memory problems and also supports carers/families.

Referral by: GP

PSYCHIATRIST

A psychiatrist will provide medical supervision for the person with mental health/memory problems if required, and gives diagnosis of specific condition.

HOSPITALS

Brecon, Llandrindod Wells and Ystradgynlais in Powys have a mental health ward for older adults. These provide assessment for patients with specific nursing care needs.

Dementia Home Treatment Team (DHTT)

Following the temporary closure of Fan Gorau ward in Newtown hospital a DHTT been established in North Powys. This provides intensive support to patients in their own homes, residential and nursing homes and community hospitals.

DAY HOSPITALs

Provide a multi-disciplinary mental health/dementia assessment for a set period, transport available.

OCCUPATIONAL THERAPIST

Occupational therapists provide assessment and advice for practical day-to-day activities at home.

INDEPENDENT ADVOCATE

Advocates provide advocacy for those with mental health/ memory problems, ensuring human rights and dignity are upheld and respected – guided by mental health legislation.

CONTINENCE ADVISER

These advisers provide a thorough assessment of continence problems and causes, and arranges suitable support and resources – home visits or out-patient appointments. Referral by: Self or any other agency.
